Terms and Conditions:

  • You must be aged 18 to 70 years old.
  • Review of your eligibility is done every 15th of the month.
  • Your enrollment and the effectivity of this plan will commence once 20 SMAC points (SMAC members) or 40 SMAC points (SM Prestige members) have been debited from your card.
  • Renewal and re-enrollment to this plan is automatically done after each year of membership.

 

This is a group insurance plan. No Certificate of Coverage (COC) is necessary when claiming.
